Job summary

St. John's Community Health is seeking a Behavioral Health Clinician II to provide screening, counseling and therapy within an integrated medical and behavioral health setting in California.

The role requires an LCSW or LMFT licensure and a collaborative, self-motivated approach to care. You will assess and treat patients across sites, coordinate referrals, and participate in multidisciplinary teams to promote wellness.

Qualifications

• Master’s Degree in Social Work, Marriage & Family Therapy, Clinical Psychology, or other discipline acceptable toward licensure by the California Board of Behavioral Sciences as a therapist.
• 2 years of direct mental health services in an integrated health setting.
• California-licensed LCSW or LMFT is required.
• Bilingual ability is preferred.

Responsibilities

• Provide assessment, evaluation and mental health treatment.
• Provide individual and group therapy to promote overall wellness, in accordance with the treatment plan.
• Consult with medical providers on psychosocial issues patients bring to their medical appointments.
• Triage both urgent and routine referrals from medical providers and program staff.
• Collaborate with staff and providers to schedule patients for intake.
• Engage in regular case conferencing with program staff and primary care departments.
• Formulate client-centered treatment plans for assigned clients.
• Ensure linkage to case management, advocacy, medication monitoring, emergency services, insurance companies, education referrals and consultation with other agencies as appropriate.
• Collaborate with primary care, prescribing physician, emergency services, program staff, and consult other agencies regarding each client.
