Job summary

Qualifications

• Provides focused quality clinical services as appropriate to model of care, clinic setting, and patient’s presenting health and behavioral concerns.
• Utilizes a variety of intervention and psychotherapeutic approaches with active development of behavioral change planning.
• Assesses patient’s mental/behavioral health related conditions utilizing DSM, PHQ-9, and other screening tools for treatment planning.
• Develop patient treatment plans appropriate for services and collaborate with patients to set goals and timelines.

Responsibilities

• Provides on-site crisis assessment, triage and intervention coordinating with community organizations and hospitals.
• Maintains accurate electronic patient records in compliance with HIPAA and organizational policies.
• Attends provider meetings and participates in ongoing training related to Quality Improvement.
• Establishes collaborative relationships with medical/behavioral health teams to promote integrated care.
