Responsibilities

Engages with high-risk, high-utilizing behavioral health patient populations.
Collaborates with treating primary care providers and multidisciplinary teams to plan and support treatment.
Advocates for clinically appropriate care and escalates concerns as needed.
Coordinates and facilitates educational sessions related to psychosocial education.
Maintains patient privacy and adheres to laws, regulations, and Kaiser Permanente policies.
Tracks referrals and outcomes for quality management.
Connects patients with internal or external resources and follows up as appropriate.
Assists with scheduling and participation in multi-disciplinary case reviews.
