Job summary

Psych Health Associates is seeking a Behavioral Health Coordinator to provide on-site behavioral health coordination for patients in the Behavioral Health Integration program across multiple post-acute facilities in Bradenton, FL. Qualifications

Experience in healthcare, behavioral health, care coordination, or related patient-facing work.
Ability to manage a high-volume caseload and navigate multiple facility environments.
Proficiency with electronic health records (EHR) and digital communication tools.
Strong communication, organization, critical thinking, and follow-through.
Demonstrated cultural competency and ability to work with diverse populations.

Responsibilities

Manage a caseload of patients enrolled in the behavioral health program as per CMS/VBC guidelines.
Perform monthly behavioral health screenings as required by PHA protocols.
Identify and escalate changes in condition or safety concerns to the BHCM or clinical team.
Support coordination of additional behavioral health services when indicated.
