Qualifications

• Experience in Collaborative Care Model or integrated behavioral health preferred.

Responsibilities

• Provide clinical care management and follow-up contact.
• Monitor care access wait times to ensure timely access to services.
• Interact with patients, families, clinicians, and staff regarding treatment implementation.
• Coordinate with external mental health facilities/providers to ensure continuity of care.
