The Care Center Specialist PRN is responsible for receiving, scheduling and referring inquiry calls, coordinating assessments and intake paperwork, and guiding the admission process. The role emphasizes assisting in coordinating admissions with the Case Manager, Coordinators and Director, while supporting smooth transitions for patients and families.

Qualifications

• Bachelor’s Degree in Human-related field preferred.
• Experience working in a hospital intake department or business office preferred.
• C.N.A. preferred.

Responsibilities

• Receive referral calls, obtain minimal demographic information, and refer the call to the appropriate source or schedule an assessment.
• Process all requests for information and external referrals.
• Completes pre-certifications as needed.
• Assist in arranging for admission of patients, including alerting the business office staff to begin the verification of benefits, initiating calls to physicians and other related tasks as directed by the Case Manager.
