Job summary

Transition Home Healthcare and Pathways Hospice in Houston seeks a self-driven Clinical Intake Specialist to ensure outstanding admissions experiences for patients and families from the start.

You will coordinate with Case Managers and healthcare providers, educate clients about home health and hospice services, review patient charts for clear communication, and serve as the primary contact throughout the admissions process to ensure coordinated care.

Qualifications

• 1 year of Intake experience (Home Health/Hospice).
• Ability to multi-task with high-volume referrals.
• Insurance verification and multi-hospital portals experience.
• Proficient in Word, Excel, PowerPoint, and Outlook.
• LVN is a plus but not mandatory.
• Experience with HCHB EMR.

• Providing an outstanding admissions experience for our patients.
• Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers.
• Providing education to patients and family members regarding their home healthcare experience.
• Reviewing patient charts and ensuring information is clearly communicated to the patients care team.

• Ensuring coordination of care with other team members.
