SHYS HEALTH, formerly known as S&H Youth and Adult Services, Inc, in Salisbury, NC, seeks an Intake Specialist to join our new management team. The ideal candidate is organized, detail-oriented, self-driven and engaged.

The role involves conducting clinical assessments, coordinating treatment plans, and ensuring timely access to services for adults, adolescents and children. Shift Mon–Fri 8:30AM–5:30PM; remote work option may be available.

Qualifications

• Master's degree in Human Service field with 2 years post-graduate licensure experience.
• Possession of a license or associate license.
• 18 years or older with ability to read, write and follow instructions.
• Experience counseling, crisis intervention, and case management with adults, families.

Responsabilités

• Conduct diagnostic clinical assessments identifying problems, history, service needs and goals for adults, adolescents and children.
• Develop and coordinate new patient treatment plans and crisis plans including scheduling for primary care, therapy, psychiatry, and peer support.
• Conduct follow-up calls to promote engagement and assess risk and safety until clients are engaged in services.
• Provide triage and crisis intervention to facilitate necessary services including inpatient hospitalization when indicated.
• Conduct and maintain weekly outpatient therapy for up to 15 individuals.

• Clinical Supervision and Training for Licensure/Certification. Clinical Supervision for licensure/certification is offered weekly as a benefit of your employment. That time is not paid. Participation in the any of the training and supervision for licensure and certification will require a two-year employment commitment from the commencement date of licensure, certification, and/or supervision and training. Responsibilities

1. Conducts diagnostic comprehensive clinical assessments to identify presenting problems, biopsychosocial history, substance use history, status of social determinants of health, service needs, and life goals for adults, adolescents and children.
2. Develop, coordinate and implement new patient treatment plan and crisis plan including coordination of first time appointments for primary care, therapy, psychiatric appointments, peer support and other mental health and substance use treatment services.
3. Conduct follow up calls to promote engagement and assess for risk and safety until client is engaged in services; development of natural supports, and other support as needed.
4. Provides triage and crisis intervention to new patients and existing patients to facilitate necessary services such as inpatient hospitalization, when indicated.
5. Conducts and maintains weeekly outpaitent therapy for up to 15 individuals.

Qualifications

• Master Degree in Human Service Field with 2 year post graduate experience with License or Associate License AND
• Is 18 years of age or older with the ability to read, write and understand how to follow instructions and is Licensed with one year of experience with target population AND
• Has no violent criminal record orsubstantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry
• Experience providing counseling, crisis intervention and case management to adults, with severe and persistent mental illness and substance use disorders as well as children/adolescent with severe emotional disorders, substance use and their families.
• Flexibility with scheduling to meet the needs of patients Shift: Mon - Fri (8:30AM – 5:30PM) REMOTELY
