Job summary

College of Pastoral Supervision and Psychotherapy is seeking a coordinator to accurately enter patient information into EMR, schedule PT and SN admissions, and coordinate home health visits in a timely, compliant manner.

The role includes monitoring adherence to therapy assessments, supervising visits, auditing requirements, and triaging intake calls. Ideal candidates have a High School diploma and 2–3 years of Home Health experience with strong computer skills.

Qualifications

• Excellent computer skills required.
• Associates Degree preferred.

Responsibilities

• Accurately enters all appropriate patient information into EMR.
• Schedules all PT and SN admissions and evaluations.
• Coordinates scheduling of all home health visits.
• Monitors compliance with Therapy Functional Assessments and supervisory visits.
• Monitors staff productivity and communicates with Clinical Managers.
• Completes any required audits.
• Triages Intake phone calls and referrals.
• Assists the Intake Department as needed.
• Communicates request for authorizations with appropriate personnel.
