Job summary

College of Pastoral Supervision and Psychotherapy is seeking an Intake Coordinator to accurately enter patient information into the EMR and coordinate admissions and home health visits. You will support Therapy Functional Assessments, supervise staff productivity, and liaise with Clinical Managers.

The role requires a high school diploma, associates degree preferred, and 2-3 years of home health experience with strong computer skills.

Responsibilities

• Enter patient information into EMR accurately.
• Schedule PT and SN admissions and evaluations.
• Coordinate scheduling of home health visits.
• Monitor compliance with Therapy Functional Assessments and supervisory visits.
• Monitor staff productivity and communicate with Clinical Managers.
• Complete required audits.
• Triages Intake phone calls and referrals.
• Assist Intake Department as needed.
• Communicate requests for authorizations with appropriate personnel.
