Oceans Healthcare in Texas is seeking an Intake Clinician to respond to inquiry calls, perform initial intake assessments, and coordinate admissions for behavioral health services.

The role requires an active RN license and at least 1 year of mental health/chemical dependency experience with age and disability populations. Strong communication, rapport-building, and accurate documentation are essential for success.

Qualifications

• Active RN license in the practicing state.
• Minimum 1 year experience in Mental Health/Chemical Dependency with age/disability populations preferred.
• Ability to develop rapport with patients, families and referral sources.
• Knowledge of growth and development across patient populations.
• Knowledge of DSM-IV-R diagnosis and clinical concepts.
• CPR certification and Oceans-approved behavioral health de-escalation training.

Responsibilities

• Responds to inquiry calls, completes inquiry forms, schedules assessments and documents follow-up.
• Conducts assessments at facility or other locations and completes initial triage.
• Refers patients to appropriate resources when not suitable for facility programs.
• Coordinates pre-certification and admissions documents and UR information.
• Delivers patient services with dignity and promotes patient satisfaction.
• Identifies safety needs and reports concerns to clinical staff.
• Identifies mobility and equipment needs for patients.
• Assesses age and disability needs to determine level of care and referrals.
• Communicates with patients and families considering age, disability and cultural needs.
• Performs other duties as assigned.
