Role Description

The Intake RN is responsible for completing clinical assessments for individuals seeking behavioral health services, including gathering medical and psychosocial histories, evaluating risk factors, and determining appropriate levels of care. This role coordinates admissions and transfers, collaborates with physicians and interdisciplinary team members, and ensures accurate documentation in accordance with regulatory and facility standards. The Intake RN provides compassionate, trauma-informed communication with patients, families, and referral sources, while helping to facilitate smooth transitions into inpatient or outpatient programs. This is a full-time, on-site role located in Clive, IA, requiring flexibility to work evenings, weekends, or holidays as needed to support 24/9 operations.

Qualifications

• Active Registered Nurse (RN) license in the state of Iowa (or eligibility for licensure); graduation from an accredited nursing program.
• Experience in behavioral health, psychiatric nursing, or emergency/triage settings, with strong assessment and clinical decision-making skills.
• Proficiency in patient intake, triage, and admission processes, including completion of nursing assessments, risk screenings, and care coordination.
• Strong communication and interpersonal skills to interact effectively with patients, families, referral sources, and multidisciplinary team members.
• Ability to maintain accurate, timely electronic health records and documentation, with attention to regulatory and compliance requirements.
• Demonstrated commitment to person-centered, culturally sensitive, and non-discriminatory care in a fast-paced environment.
• Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) and Crisis Prevention Intervention (CPI) or equivalent de-escalation training preferred.
• Ability to work rotating shifts, including evenings, nights, weekends, and holidays, and to manage multiple priorities safely and efficiently.
