Job summary

Southridge Behavioral Hospital is seeking an Intake Registered Nurse to conduct telephonic and in-person risk assessments and support admissions under the Intake Director. This role partners with referral sources, families and unit staff to ensure appropriate level of care.

Shift is full-time from 7pm to 7:30am with weekend rotation; located in Grand Rapids, MI area. Requires Michigan RN license and at least 3 years in psychiatric care.

Qualifications

• Graduate from an accredited professional nursing program.
• A minimum of three years’ experience in a psychiatric health-care facility preferred.
• Registered Nurse (RN) license issued by the State of Michigan required.

Responsibilities

• Provide telephonic and in-person risk assessments to help identify appropriate level of care.
• Coordinate referral and intake processes and facilitate admissions.
• Maintain communication with referral sources, families, patients and unit staff.
• Collaborate with Business Office for patient financial data and insurance verification.
• Coordinate with UR for certification of admitted patients.

Position Description

The Intake RN, under the supervision of the Intake Director, provides telephonic and in‑person risk assessment to assist the physician in identification of appropriate level of care for potential patients. The Intake RN is the initial contact with referral sources and potential patients, coordinates the referral and intake process, facilitates the admission process and maintains communication with referral sources, families, patients, and unit staff. The Intake RN works with the Business Office regarding patient financial data and verification and prior authorization of insurance benefits as needed, as well as with the UR Department for the certification of patients admitted.

Hours and Location

Full Time, 7pm to 7:30am for Night shift. Weekend rotation required.

Qualifications
