Job summary

Southridge Behavioral Hospital, a joint venture with Trinity Health System and Universal Health Services, is seeking an Intake Clinician to perform telephonic and in-person risk assessments and coordinate referrals. Under the supervision of the Intake Director, you will manage the intake and admission processes while communicating with referral sources, families, patients, and staff.

The role requires a Master’s Degree in Social Work, Michigan licensure (MA level) or higher, and at least 3 years

• Experience in psychiatric health-care settings preferred.
• Ability to perform telephonic and in-person risk assessments.
• Strong communication with referral sources, families and staff.

Responsibilities

• Provide telephonic and in-person risk assessments to identify appropriate level of care for potential patients.
• Coordinate referral and intake process and facilitate admissions.
• Communicate with referral sources, families, patients and unit staff; maintain documentation.
• Collaborate with Business Office for patient financial data and insurance pre-authorizations; coordinate with UR for admissions certification.

Position Description

The Intake Clinician, under the supervision of the Intake Director, provides telephonic and in-person risk assessment to assist the physician in identification of appropriate level of care for potential patients. The Intake Clinician is the initial contact with referral sources and potential patients, coordinates the referral and intake process, facilitates the admission process and maintains communication with referral sources, families, patients, and unit staff. The Intake Clinician works with the Business Office regarding patient financial data and verification and prior authorization of insurance benefits as needed, as well as with the UR Department for the certification of patients admitted.

Hours and Location

Full Time, 7pm to 7:30am. Weekend rotation required.

Qualifications

Job Requirements

Education: Masters Degree required.

Experience: A minimum of three years’ experience in a psychiatric health-care facility preferred.

License: Active MA Level License in the state of Michigan: LMSW, LLMSW, LLPC, LPC, LLP, TLLP
