Job summary

Comfort Health in Rochester, MN seeks an Intake/Referral Coordinator to manage client admissions, verify eligibility, and coordinate care start dates. You will ensure accurate patient records and collaborate with the clinical team for timely authorizations and referrals.

The role requires strong scheduling skills, Medicare/Medicaid knowledge and the ability to learn new software quickly. Full-time, on-site position with benefits and a focus on compliant, compassionate care.

Qualifications

• Excellent working knowledge of primary home health diagnoses and related interventions and care protocols.
• Medicare and non-Medicare reimbursement requirements including eligibility for service.
• Scheduling of client services within specified discipline/frequency parameters.
• Ability to quickly learn new software applications.

Responsibilities

• Receives referrals from physicians, facilities and families and initiates the intake process.
• Coordinates the start of care date and/or discharge date.
• Enters new client records in the agency system ensuring accuracy of all client demographic information.
• Performs eligibility/insurance verification.
• Coordinates with the Clinical Supervisor, Billing Manager and clinical team to obtain required information for client records.
• Obtains authorizations for service requests from non-Medicare pay sources and ensures authorizations are in place for first and subsequent episodes.
• Attends regular interdisciplinary team meetings for admissions and recertifications.
• Supports Scheduler and assists with scheduling changes.
• Schedules and participates in on-call assignments in cooperation with the Clinical Supervisor.
• Answers the phone.
• Adheres to HIPAA and client information privacy requirements.

Salary Range: $52,000.00 To $62,000.00 Annually

Comfort Health, locally owned and operated for over 40 years, is currently accepting applications for an Intake/Referral Coordinator. This position is responsible for the admission process of new clients by ensuring availability of reimbursement for services, identifying the clinical team to deliver care, and scheduling client visits within prescribed discipline and visit frequency requirements.

Qualifications

• Excellent working knowledge of primary home health diagnoses, related interventions and care protocols.


• Demonstrable working knowledge of Medicare and non-Medicare reimbursement requirements including eligibility for service.


• Demonstrable experience with scheduling of client services within specified discipline/frequency parameters.


• Ability to quickly learn new software applications.

Essential Functions and Responsibilities:

• Receives referrals from physicians, facilities. and families and initiates the intake process.


• Coordinates the start of care date and/or discharge date.


• Enters new client records in the agency system ensuring accuracy of all client demographic information.


• Performs eligibility/insurance verification.


• Confers with the Clinical Supervisor as to the preliminary eligibility for service.


• Coordinates with the Clinical Supervisor, Billing Manager and clinical team to ensure required information is obtained for client records.


• Receives documentation and scans the records into the agency system per company policies.


• Obtains authorizations for service requests from non-Medicare pay sources and ensures that such authorizations for service are in place for the first and subsequent episodes or treatment periods.


• Attends regular interdisciplinary team meetings for purposes of identifying new admissions and recertifications.


• Supports Scheduler and assists with as needed scheduling changes.


• Schedules and participates in on-call assignments in cooperation with the Clinical Supervisor.


• Answers the phone.


• Adheres to HIPAA and client information privacy requirements.
