Job summary

Enhabit Home Health & Hospice is seeking a Referral Coordinator to coordinate patient referrals and serve as a resource for referral sources. You will collaborate with the branch and sales teams to ensure smooth coordination and transition to services, while applying regulatory guidelines governing home health and hospice care.

The role requires at least two years in health care with one year in home health or pediatrics, strong customer service experience, and knowledge of Medicare guidelines;

Qualifications

• Must have high school diploma or equivalent.
• At least two years of experience in health care and one year in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have knowledge of Medicare guidelines that govern home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and act as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure timely coordination and transition to services.
