Job summary

Enhabit Home Health & Hospice is seeking a coordinated intake professional to manage patient referrals and support transitions to services. You will collaborate with branch and sales teams to ensure smooth coordination and apply regulatory guidelines in daily work.

The role requires a high school diploma and 2+ years in healthcare, with Medicare guideline knowledge and basic mobile tech skills. A valid driver license and reliable transportation are expected.

Qualifications

• Must possess a high school diploma or equivalent.
• Two years of experience in the health care industry and one year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills including operation of a mobile device.
