Job summary

The State of Florida's Agency for Persons with Disabilities is seeking an Intake Coordinator to provide intake informational support to individuals with developmental disabilities and determine APD eligibility. The role maintains caseloads, guides applicants through the enrollment process, and coordinates documentation.

Requires travel within the region, strong data handling in iConnect, and ability to explain the process via phone, email, or walk-ins.

Qualifications

• Valid Florida Driver License or reliable transportation.
• Ability to travel for work.
• Experience with Developmental Disabilities or Mental Illness or related fields.
• Preference for fluent bilingual - Creole, Spanish speaking and writing.

Responsibilities

• Complete the initial screening form and explain the intake process via phone, email, mail, or walk-ins.
• Process APD application packets and document the process in the system tracker.
• Conduct quality review of documents to determine eligibility for APD services.
• Update progress notes and generate required eligibility notices in iConnect.
• Initiate and complete the short version support plan after eligibility is determined.
• Maintain referral and applicant information in iConnect and other systems.
• Review high-risk case files to ensure continuity of services for users aged 3–5 years and older.
• Perform other related duties or assignments.
