Interim HealthCare in Grand Rapids, MI is hiring an Intake Nurse (RN or LPN) for a full-time, Monday–Friday office position. The role focuses on patient advocacy, intake coordination, and initial admission screening, with a patient-centered, team-driven culture.

Qualifications

• Graduate of an accredited school of nursing and current RN or LPN licensure in Michigan.
• Minimum of 1 year of RN or LPN clinical experience.
• Home health care experience preferred.

Responsibilities

• Act as intake liaison interfacing with referral sources, health professionals, and field staff.
• Receive and coordinate incoming calls, providing information to patients and referrals.
• Inform patients and families about care initiation and service arrangements.
• Complete intake screening and make preliminary admission decisions.
• Log referrals, prepare reports, and provide telephonic contacts as needed.
• Promote care/services to prospective patients/clients and accept verbal orders when permitted.
