Job summary

Fundaciones Integrales in Arkansas is seeking an intake nurse (LPN or RN) to identify client needs and coordinate personal care services. The role includes timely intake completion, communication with office and scheduling staff, and developing care plans after initial assessments.

Must have a clear Arkansas license and strong documentation skills. Experience in home care is preferred, with ability to work in an office setting, maintain records, and participate in weekly staff meetings.

Qualifications

• Must hold LPN or RN license in Arkansas.
• Clear criminal background screening required.
• Experience in a home care setting preferred.
• Must be computer literate and able to maintain simple records.

Responsibilities

• Identifies and evaluates client needs and appropriateness of personal care services.
• Intake nurse to make initial contact and complete intake forms within set timelines.
• Notify Office Manager and Scheduling Team after intake completion.
• Communicate with Office Manager and Scheduling Team about client needs.
• Complete intakes for Medicaid/PASSE clients discharged from hospital within 48 hours.
• Develop Care Plan after initial assessment and consultation with client or responsible party.
• Document status changes to Office Manager immediately.
• Attend weekly staff meetings.
