Job summary

Fundaciones Integrales is seeking a licensed LPN or RN in Arkansas to support intake processes and coordinate care for clients in their homes. The role requires travel to client homes and collaboration with the Office Manager and Scheduling Team to ensure timely intake and appropriate documentation.

Bilingual ability is a plus and experience in home care is preferred. The position involves completing initial assessments, developing care plans, and ensuring all required PA paperwork and updates

Qualifications

• Licensed as LPN or RN in Arkansas in good standing.
• Clear criminal background screening.
• Experience in a Home Care setting preferred.
• Must be computer literate and able to maintain simple records.
• Position requires travel to and from client homes and possibly training travel.
• Bilingual is helpful.

Responsibilities

• Completes intake forms upon client admission and notifies Office Manager and Scheduling Team; intake within 48 hours of referral and admission paperwork within 24 hours of intake.
• Communicates with Office Manager and Scheduling Team regarding client needs and/or requests.
• Completes intakes on Medicaid and/or PASSE clients discharged from hospital or facility within 48 hours of returning home; ensures all supporting documentation for reassessment is in the client’s chart.
• Supervisory Home Visits are to be completed as follows: AR Choices every 6 months rotating with aide present and not present; VA monthly or as authorized; enter PA requests and develop Care Plans after initial assessment.
