Job summary

Delta Community Supports is seeking an Intake Specialist to manage referrals and the end-to-end intake process for individuals seeking developmental disability services and community supports.

You will determine eligibility, coordinate resources, and help individuals transition into programs that foster independence and growth. This role reports to the State Director of Community Living (NJ).

Qualifications

• A high school diploma or GED is required.
• An associate degree or 60 college credits is acceptable; bachelor’s preferred.
• 2–3 years of relevant experience in social services is desirable.

Responsibilities

• Handle end-to-end intake referrals and eligibility assessments.
• Coordinate intake interviews with individuals and families.
• Prepare intake packets and gather required documentation.
• Assist with Medicaid waivers enrollment and related paperwork.
• Maintain client records in Evolv database securely.
• Coordinate logistics for program admissions and housing needs.
• Represent the agency at outreach events and educate stakeholders.
