Job summary

Quail Run Hospital in Phoenix seeks an Intake Counselor to perform risk assessment and crisis intervention, both by phone and in person. The role supports referral sources, coordinates the intake process, and assesses level of care to facilitate admissions.

The Intake Clinician also assists physicians with face-to-face assessments, including illness history, family dynamics, and psychosocial information for patients across units.

Qualifications

• Master's degree in Counseling, Psychology, Human Services, or MSW required.
• Licensure through the Arizona Board of Behavioral Health Examiners preferred.
• Experience in intake, assessment, admission, and crisis intervention with psychiatric populations, preferably inpatient.
• Must have or obtain level one fingerprint clearance card.

POSITION SUMMARY

The Intake Counselor provides telephonic and in person risk assessment and crisis intervention, initial contact with referral sources and potential patients, coordinates the referral and intake process, determines appropriate level of care, facilitates the admission process and maintains communication with referral sources, families, patients and unit staff. The Intake Clinician also assists the Physician by providing face to face clinical assessments, including the history of presenting illness, family dynamics and supporting psychosocial information.

Qualifications

Minimum Requirements:

• Master's Degree in Counseling, Psychology, Human Services field, or Masters in Social Work required.
• Licensure through the Arizona Board of Behavioral Health Examiners preferred.
• Prior intake, assessment, admission, and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting.
• Must have or be able to obtain level one fingerprint clearance card.

Quail Run Hospital is a 102-bed, acute care psychiatric hospital located in Phoenix, Arizona.
