Job summary

Catholic Charities Brooklyn and Queens is seeking an Intake Coordinator to review Case Management services and screen older adult callers for eligibility. You will input data to initiate intake and enrollment in the NYC Aging database after consent, and explain services to community callers.

Responsibilities include assessing eligibility, maintaining resources, providing information and referrals, and submitting monthly reports.

Qualifications

• Bachelor’s Degree in Human Services or related field.
• Knowledge of entitlements for older adults preferred.
• Good verbal and written command of English.
• Proficient in Excel and Microsoft Office.
• Within 6 months learn NYC Aging client software for data entry.

Responsibilities

• Perform intake activities, assess eligibility and initiate services.
• Maintain current resource book of providers and community resources.
• Provide information and referrals and screen callers for eligibility.
• Maintain case notes and monthly statistics/reports.
• Utilize Vive data system and UNIFORM for client information.
