Job summary

Hospice Savannah is seeking an RN Intake Coordinator to schedule intake visits, coordinate home and center visits, and document medical and social information throughout enrollment. The role may include guiding tours of the PACE Center for participants and caregivers during orientation.

The ideal candidate is a licensed Georgia RN with experience with frail or elderly populations and familiarity with CMS PACE requirements, capable of independent work and effective communication.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Current RN license in good standing to practice in Georgia.
  • One year of experience with frail/elderly population or willingness to receive CMS-based training.
  • Ability to meet CMS-defined competencies before working independently.
  • Clear medical clearance including vaccines prior to participant care.
  • No criminal convictions involving Medicaid/Medicare or CMS programs.
  • Not excluded from participating in Medicare/Medicaid programs.
  • No convictions related to abuse or fraudulent activity.

The coordinator may also provide tours of the PACE Center as part of participant and caregiver orientation.

QualificationsMinimum Qualifications
  • Graduate of an accredited School of Nursing.
  • Current RN license in good standing to practice in Georgia
  • One year of experience working with a frail or elderly population or, in the absence of such experience, the ability to receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently (CMS required).
  • Ability to meet a standardized set of competencies for the specific position description established by PACE before working independently (CMS required).
  • Medical clearance for communicable diseases and up-to-date immunizations and vaccines prior to engaging in participant care (CMS required).
  • No convictions of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs under Title XX of the Act (CMS required).
  • Not excluded from participating in Medicare and Medicaid programs (CMS required).
  • No convictions of criminal offenses related to physical, sexual, drug, or alcohol abuse (CMS required).
Competencies, Preferred Knowledge, Skills, and Abilities
  • Experience working in Electronic Health Records (EHRs) (Preferred).
  • BLS certification (Preferred).
  • Knowledge of geriatric care principles and the needs of frail older adults.
  • Understanding of Medicare, Medicaid, and PACE regulations.
  • Strong assessment and documentation skills.
  • Effective verbal and written communication skills.
  • Proficiency with Electronic Health Records and basic office technology.
  • Ability to work independently, prioritize tasks, and manage intake workflows.
  • Ability to engage compassionately with participants and caregivers from diverse backgrounds.
  • Working knowledge of PACE marketing and enrollment communication requirements, including use of CMS-approved materials, avoidance of misleading information, and adherence to prohibited marketing practices.
  • Ability to communicate clear about PACE enrollment and disenrollment policies, covered services, and participant obligations, and to route grievances/appeals questions to the appropriate organizational process.
  • Ability to recognize and promptly escape potential compliance concerns related to enrollment activities, documentation, privacy/confidentiality, or outreach/marketing practices.
Physical Demands and Work Environment

This role requires the ability to travel to participant homes, hospitals, skilled nursing facilities, and community locations throughout the service area. Work is performed in a combination of office, clinic, and participant home environments.

The position requires the ability to sit, stand, walk, and drive for extended periods; maintain visual focus for computer-based documentation; and communicate effectively in person, by phone, and in writing.

The employee must be able to lift, carry, or move up to 75 lbs, such as educational materials, portable assessment tools, or small equipment during home visits. Occasional bending, reaching, and navigating varied home environments—including stairs, uneven surfaces, and tight spaces—may be required. The work environment involves regular exposure to typical home conditions and occasional exposure to environmental factors such as pets, odors, varying temperatures, and household clutter. The role may also include intermittent exposure to individuals who are ill or have compromised mobility or cognitive limitations. Standard infection control and safety precautions must be followed at all times.

Position Type and Expected Hours of Work

This is a full-time position, non-exempt position. Standard schedule is Monday–Friday, 8 a.m.–4:30 p.m., with flexibility based on participant needs, home visit schedules, and organizational priorities. Additional hours may be required during periods of high enrollment volume or weekend coverage need.

Travel

Travel is required for home visits, community outreach, eligibility assessments, and meetings. Typical travel is within PACE Georgia catchment area and the Savannah region. Occasional travel for training or organizational meetings may be required
