The role requires a Bachelor's degree in psychology, social work, counseling, or RN/LVN, plus at least one year in mental health/chemical dependency care. CPR and de-escalation training are expected.

Qualifikationen

• Bachelor's degree in Psychology, Social Work, Counseling or related field or LVN/RN or equivalent experience.
• Minimum of one year experience in Mental Health/Chemical Dependency field serving an Age/Disability Specific population.
• Must successfully complete CPR certification and Oceans approved behavioral health de-escalation program.

Aufgaben

• Responds to inquiry calls, completes inquiry forms, schedules assessments and documents follow up.
• Conducts assessments at facility or other locations and completes initial triage.
• Refer patients to appropriate resources if not suitable for facility programs.
• Coordinates pre-certification and admissions processes and handles necessary forms and documentation.
• Delivers patient services with respect, promoting dignity and satisfaction.
• Identifies safety needs and reports concerns to clinical staff.
• Assesses age/disability-related needs to determine level of care and referrals.
• Communicates with patients and families considering age, disability, and cultural needs.
• Performs additional duties and projects as assigned.
