The Intake Coordinator – Home Infusion Referrals is a full-time, on-site role based in Chandler, AZ. This position is responsible for receiving, reviewing, and processing home infusion referrals from healthcare providers and facilities, ensuring all required documentation and orders are complete and accurate. Day-to-day tasks include verifying insurance benefits, obtaining prior authorizations, and coordinating with case managers, prescribers, and internal pharmacy and nursing teams to support timely initiation of therapy. The Intake Coordinator communicates with patients and caregivers to explain services, confirm demographic and insurance information, and address basic questions regarding the referral and intake process. This role also maintains organized records, updates electronic systems, and follows established protocols to support compliance, efficiency, and a positive experience for patients and partners.
