What To Expect (Job Responsibilities)

• Serve as the initial point of contact for potential home health patients, coordinating with Home Health nurses
• Manage new referrals and ongoing follow-ups, acting as a liaison between patients, families, and referral sources
• Ensure a seamless intake process for patients and their families
• Exercise discretion and make independent judgments while adhering to established guidelines

What Is Required (Qualifications)

• High School Diploma or GED
• Minimum of 2 years of recent relevant experience in a healthcare setting
• Knowledge of medical terminology, including CPT, HCPCS, and ICD-9 coding
