Job summary

Tanner Health is seeking an Intake Coordinator for Home Health to accept telephone referrals and coordinate initial in-home assessments. You will be the first point of contact for agencies, hospitals, and physician offices, coordinating patient care and referrals efficiently.

The role requires strong organizational and communication skills, plus proficiency with healthcare systems like Maestro and Meditech. Prior intake or scheduling experience in healthcare is preferred.

Responsible for accepting telephone referrals and coordinating initial in-home assessments for patients that are referred to Home Health. This individual will be the first point of contact for the agencies and will be responsible for communicating and coordinating patient care with hospitals, physician offices, and other referral sources.

Education

High School Diploma plus 1 year of vocational school

Experience

One year of related experience. Requires a working knowledge of standard practices and procedures.

Qualifications

• Good organizational skills are required.
• Good verbal and written communication skills are required.
• Must have proficient computer skills. Experience with Maestro and Meditech clinical application systems preferred. Prefer a minimum of two years of experience working in healthcare with knowledge of in-take and patient scheduling and insurance verification.
• Will consider GED with 1+ years of vocational school and 1+ years of medical office experience.
