Job Opportunity: Intake Coordinator - West Michigan

Responsibilities:

1. Manage pre-screen eligibility process: Conduct eligibility screening for program applicants through referrals, phone calls, and walk-ins. Determine and follow eligibility guidelines according to program policies and procedures.
2. Documentation: Provide all required documentation to outside agencies supporting the beneficiary file (probation, parole, courts, etc.).
3. Community Referral and Data Management: Be knowledgeable of community referral agencies, communicate intake policies effectively, and enter and maintain relevant applicant information in the online records database, ensuring files are timely, accurate, and complete.
4. Work Schedule: This position requires availability in the evenings and some weekends.
