The Intake Coordinator is responsible for managing the intake and referral process for new patients entering home health services. This role coordinates referrals, verifies information, ensures documentation is complete, and collaborates with clinical and administrative teams to facilitate timely admissions in compliance with Medicare Conditions of Participation, state regulations, and agency policies.
This position reports to the Clinical Manager, Director of Nursing (DON), Administrator, or designated leadership responsible for intake and operations. The Intake Coordinator works closely with clinical staff, referral sources, and administrative departments.
This role is administrative and coordination-focused and does not provide direct patient care or clinical assessments. The Intake Coordinator is responsible for processing referrals, coordinating admissions, and ensuring documentation and eligibility requirements are met prior to patient admission.
