Qualifications

• Experience in healthcare intake, admissions, or referral coordination.

• Excellent communication and customer-service skills with patients and providers.
• Proficiency with electronic health records (EHR) or intake software systems.

Responsibilities

• Referral Management: receive, review, and process new patient referrals and verify required authorizations.
• Nurse pairing & case assignment: match referrals with RNs by geography, skill, and availability; ensure orders are complete.
• Care coordination: communicate referral status and patient readiness with nursing and scheduling teams.
• Documentation: enter and maintain patient data, authorizations, and notes in EHR.
• Collaboration: support smooth intake to active care transitions with the team.
• Communication: maintain professional, proactive communication with patients, families, and providers.
• Compliance: ensure intake processes meet HIPAA and regulatory standards.
