Job summary

Medstaff Group in Hempstead, NY is seeking an Intake Coordinator to support a Long Island care management program. The role requires Spanish language skills and involves guiding individuals and families through services, insurance, referrals, and enrollment.

You will maintain organized records and ensure compliance with program requirements. During your initial months, you’ll also serve as a Care Manager, conducting monthly home visits and providing ongoing support to members and families.

Qualifications

• Bachelor’s degree required.
• Spanish bilingual skills required.
• Strong organizational, communication, and computer skills.
• Detail-oriented, dependable, and able to work independently and as part of a team.
• Comfortable communicating with families and working through paperwork.
• Experience in healthcare, social services, psychology, case management, government programs, or insurance is a plus.

Responsibilities

• Coordinate the intake process, including reviewing referrals, assessing eligibility, and supporting enrollment.
• Conduct monthly home visits and provide care management support.
• Work with families, healthcare providers, therapists, community organizations, and Medicaid Managed Care Plans.
• Conduct assessments and develop individualized care plans.
• Maintain accurate, timely documentation.
• Follow applicable New York State Department of Health, Health Home, and regulatory requirements.
• Attend weekly virtual trainings, team meetings, and program updates.
• Help maintain a current directory of services, providers, and community resources to support appropriate referrals.

Location: Hempstead, NY
Job Type: Full-time, starting part-time
Pay: $25–$29 per hour (approximately $27 per hour)
