Job summary

Suffolk Primary Health, LLC is seeking a warm, organized Spanish-speaking Intake Coordinator for our Hempstead team to manage the care management intake process and ensure families receive the services they need.

The role involves coordinating with families, providers, and Medicaid Managed Care Plans, conducting assessments, developing individualized Care Plans, and maintaining documentation with compliance and ongoing training.

Qualifications

• Bachelor's degree required.
• Bilingual Spanish required.
• Experience with government programs or case management is a plus.
• Strong organizational and computer skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee and manage the Care Management intake process.
• Conduct monthly home visits during initial months and provide ongoing support.
• Coordinate services with families, providers, and Medicaid Managed Care Plans.
• Conduct assessments and develop individualized Care Plans.
• Maintain accurate, complete, and timely documentation.
• Ensure compliance with DOH, Health Home and applicable requirements.
• Attend weekly trainings and team meetings.
• Develop and maintain a directory of services and community resources.
