Choice Health at Home is seeking a full‑time Intake Coordinator at our Tyler, TX home health branch. You will manage initial patient referrals, verify insurance benefits, obtain authorizations, and enter patient data into EMR to ensure a seamless start of care.

The role requires documenting referrals promptly, coordinating with nursing supervisors and the interdisciplinary team, and maintaining effective communication with patients and referral sources.

Qualifications

• Minimum one year of referral intake experience preferred.
• Experience with third party payers, Medicare and Medicaid highly desirable.
• Medical field background preferred, including familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.

Responsibilities

• Receives and documents new referrals in a timely manner.
• Verifies payor sources and maintains insurance authorizations.
• Coordinates care with Nursing Supervisors, patients and interdisciplinary team.
• Communicates with patients, staff and referral sources professionally.
• Supports Agency-sponsored in-service training and QAPI participation.

A Home Health Intake Coordinator manages the initial patient referral proce ss, acting as the key liaison between hospitals, physicians, families, and clinical staff to ensure a seamless start of care . They are responsible for receiving referrals, verifying insurance benefits, obtaining authorizations, and accurately entering patient data into Electronic Medical Records (EMR).

As a Intake Referral Coordinator , you will:

Receives and appropriately documents new referrals in a timely manner.

Verifies each patient’s payor source and maintains and tracks insurance authorizations and initial certifications.

Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.

Provides effective communication to patients, staff members, other health care professionals and referral sources.

Responds to Agency and patient needs in a professional and creative manner.

Demonstrates commitment, professional growth and competency.

Participates in Agency-sponsored in-service training.

Participates in the Agency’s QAPI program.

Promotes the Agency philosophy and administrative policies to ensure quality of care.

Requirements

Minimum one year of experience in referral intake is preferred
