Job summary

Oso Home Care is seeking a detail-oriented Intake Coordinator to manage the intake process for our home healthcare infusion and pharmacy services. The ideal candidate will ensure seamless coordination between patients, healthcare providers, and our clinical team to deliver high-quality care.

Schedule is in-office, Monday to Friday, 9:30 am to 6:00 pm with rotating weekends (one to two weekend days a month).

Qualifications

• High school diploma or equivalent; associate or bachelor's degree preferred.
• Previous healthcare experience, preferably in home infusion or pharmacy services.
• Strong organizational and multitasking skills with attention to detail.
• Excellent communication and interpersonal skills.
• Proficiency in electronic health records (EHR) and Microsoft Office Suite.
• Knowledge of insurance verification and prior authorization processes.

Responsibilities

• Conduct initial patient assessments and gather medical and insurance information.
• Coordinate with healthcare providers to obtain prescriptions, orders, and histories.
• Verify insurance coverage and obtain prior authorizations as needed.
• Schedule and coordinate home infusion services and delivery of meds and supplies.
• Maintain accurate patient records in compliance with regulatory standards.
• Provide excellent customer service and support to patients and families throughout intake.
• Collaborate with the clinical team to ensure smooth transitions and continuity of care.
