Job summary

The Doe Fund Inc. in Brooklyn, NY is seeking an Intake Case Manager to welcome new clients, explain program structure, and guide them through initial interviews and intake in CARES and the TDF Database.

You will develop an Independent Living Plan, support house operations, and coordinate services with sensitivity to clients facing homelessness or justice involvement. A High School Diploma or equivalent is required, and independence with minimal supervision is valued.

Qualifications

  • Must have strong client contact skills and the ability to treat all clients with dignity and respect.
  • Ability to complete detailed intake interviews and enter data accurately into CARES and the TDF Database.
  • Independent worker capable of navigating varying situations without onsite supervision.

Responsibilities

  • Meet with every new Client to introduce program structure, guidelines, and expectations
  • Orient Client to new surroundings and provide initial bedding and toiletries
  • Complete detailed interview and paper intake with the Client
  • Enter all intake information into CARES & TDF Database
  • Construct an initial Independent Living Plan (ILP) with every new Client
  • Assist with House Management duties including digital bed management and related tasks

Key Responsibilities:

  • Meet with every new Client to introduce program structure, guidelines, and expectations
  • Orient Client to new surroundings, provide initial bedding and toiletries
  • Complete detailed interview and paper intake with the Client
  • Enter all intake information into two databases (CARES & TDF Database)
  • Construct an initial Independent Living Plan (ILP) with every new Client
  • Assist with House Management duties whenever possible, including digital bed management, urine collection for toxicology, building rounds, etc.

The Ideal Candidate:

  • High School Diploma or Equivalent
  • Experience with criminal justice or homeless population, or ability and desire to learn and adjust quickly to the population
  • Excellent Client contact skills, including a dedication to treating all Clients with dignity and respect
  • Computer Proficiency (must master two databases, and position relies heavily on email contact)
  • Must be an independent worker, and able to navigate varying situations without onsite Supervisory support
