Job summary

Springwell is seeking an Intake Assessor to help people have their voice heard about their care needs. The role is hybrid with no weekends/evenings/nights/holidays and a 37.5-hour work week at $28.21/hr.

The position conducts in-person assessments, creates care plans, coordinates services with a network of providers, and reviews community resources to empower clients. A Bachelor's degree is required, with alternative paths for experienced applicants.

Qualifications

• Bachelor's degree or higher required.
• Alternate path: Associate's degree with 3+ years of elder care or case management experience.
• Alternate path: 50% of Bachelor's coursework with plan to complete remaining within 3 years.
• Experience using computers, data entry, and generating reports.
• Must have a reliable car and valid driver's license with safe driving record.

Responsibilities

• Conduct comprehensive in-person assessments with older adults to determine needs for in-home services.
• Create care plans and coordinate services with vendor agencies as part of an interdisciplinary team.
• Review community resources and care options to empower informed consumer decisions.
• Communicate with family members, Springwell staff, and community professionals.
