Valley Behavioral Health is seeking a Social Service Worker to support the Access Team in the walk-in clinic, performing intake assessments and ASAM evaluations for adult outpatient programs including mental health, behavioral health and substance use treatment.

The role collaborates with multidisciplinary teams to connect clients with appropriate care and provides compassionate, client-centered intake support.

Qualifications

• Bachelor’s degree in Social Work or related field.
• SSW licensure in state of practice.
• CPR certification (post hire as needed).
• Internship experience as required for SSW license.

Responsabilités

• Conduct intake assessments and ASAMs for incoming clients with our Adult Outpatient programs (mental health, SUD, court ordered).
• Directs clients’ care by developing, implementing, and managing personalized care plans with specific treatment goals and objectives for each client.
• Follows agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions and discharge plans, and client care.
• Reviews and assesses care plans.
• Coordinates services among internal/external agencies and internal/external providers.
• Ensures continuity of care.
• Meets required productivity standards and other organizational expectations.
