• License must remain active and in good standing

Aufgaben

• Conduct initial and ongoing assessments to determine treatment needs and levels of care.
• Provide weekly individual counseling sessions to assigned clients.
• Facilitate individual, group, and family therapy as appropriate within licensure.
• Develop individualized treatment plans and case management goals with clients.
• Maintain accurate, timely, and compliant documentation of client progress.
• Monitor client outcomes and communicate updates to the clinical team.
• Participate in weekly treatment team meetings.
• Develop discharge and aftercare plans to support long‑term recovery.
• Liaise with insurance providers, probation officers, and external partners as needed.
• Prepare documentation for audits, accreditation, and regulatory compliance.
• Coordinate with internal departments to ensure seamless client care.
• Represent TPC Wellness in court proceedings when required.
• Contribute recommendations for program development and quality improvement.
• Attend required trainings and continuing education opportunities.
• Maintain licenses and certifications in good standing.
• Transport clients to appointments, court appearances, or program activities.
