You will assess mental health status, determine level of care, identify insurance benefits, complete admission requirements, and document in Med Series 4; a compassionate, team-based approach is essential.

Qualifications

• Ability to pass a Level 2 Background Investigation.
• Master's Degree in social work or counseling.
• Registered Intern, preferred.
• Minimum of two years experience in the mental health field preferred.
• Strong communication and teamwork.
• Must be available to attend hospital and departmental orientations.

Responsibilities

• Responds to telephone inquiries
• Knows CFBH programs and can answer general program information
• Determines initial clinical and financial eligibility
• Conducts psychosocial evaluation on each admission and gathers data
• Coordinates with the treatment team, family and patients
• Assists with discharge planning
