Qualifications

• Master’s degree in Social Work, Counseling, or an equivalent field is required.
• Experience with psychiatric and chemical dependency patients preferred.
• CPR and De-escalation certification required or obtained within 30 days of hire.
• Current Clinical or Social Work license as required by state regulations.

Responsibilities

• Conduct individual and group therapy sessions to educate patients about psychological, emotional, or substance abuse problems.
• Display active involvement in the treatment planning process.
• Provide family session counseling to patients to ensure communication and involvement of family and support groups.
• Communicate with clients, families, and outside referral sources.
• Coordinate with UR, nursing staff, and interdisciplinary team.
• Complete appropriate paperwork timely and accurately.
• Demonstrate crisis intervention and de-escalation skills.
• Assist care coordination and discharge planning as requested.
• Maintain confidentiality and comply with HIPAA and other regulations.

Responsibilities

• Conduct individual and group therapy sessions to educate patients about psychological, emotional, or substance abuse problems.
• Display active involvement in the treatment planning process.
• Provide family session counseling to all patients to ensure appropriate communication and involvement of family members and support groups.
• Actively communicate with clients, families, and outside referral sources.
• Demonstrate proactive communication with all parties involved in the patient’s treatment.
• Work well with the interdisciplinary team, including physicians, utilization review, and nursing staff.
• Demonstrate active communication with team members.
• Effectively communicate to UR about treatment and LOS issues.
• Complete appropriate paperwork for clients in a timely, accurate and complete manner.
• Demonstrate appropriate crisis intervention and de‑escalation skills.
• Act as part of the team during crisis situations, assisting others to ensure patient safety.
• Complete accurate assessments of patients utilizing clinical skills.
• Assist the care coordination department as requested.
• Complete all documentation thoroughly, timely, and in accordance with hospital standards.
• Display a thorough knowledge of confidentiality.
• Complete releases appropriately.
• Demonstrate knowledge of HIPAA regulations and abide by those standards.
• Demonstrate a working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws.
• Display active involvement in the discharge, transition, and after‑care planning treatment processes. Other duties as assigned.
