Job summary

Qualifications

• MSW or equivalent required.
• Licensed or license eligible; licensure application must be filed if eligible.

Responsibilities

• Provide psychosocial assessments and write evaluations within 48 hours of case assignment.
• Act as social worker for assigned patients; coordinate with treatment team.
• Conduct family interventions and discharge planning; document progress in chart.
• Coordinate with outpatient providers and assist in accessing community resources.
• Initiate access to services and assist with applications for relevant programs.
