Job summary

Concord Hospital is seeking a Social Worker 2 to provide advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision‑making advocacy, facilitation necessitated by life‑changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1.

Qualifications

• Masters Degree in Social Work or related field required.
• Minimum of three years clinical social work in hospital, SNF, home health or hospice.
• Demonstrates effective interpersonal, organizational and motivational skills.
• Proficient in written and verbal communication.

Responsibilities

• Conduct comprehensive assessment of patient and family needs upon consultation.
• Attend care/discharge rounds to align goals with patient/family.
• Participate in end-of-life discussions and advance care planning.
• Document outcomes and progress according to NASW ethics.
• Provide leadership to departmental staff and perform other duties.
