Concord Hospital Health System in Concord, NH seeks a Social Worker 2 to provide advanced skilled interventions for crises, care planning, and advocacy for patients and families.
The role requires a Master’s in Social Work and at least three years of hospital social work experience. Strong written and verbal communication and team collaboration are essential. This on-site position emphasizes NASW ethics and discharge planning participation.
The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1.

Masters Degree in Social Work or related field required.
Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting. Demonstrates effective interpersonal, organizational and motivational skills. Demonstrates ability to function as a team player. Proficient in written and verbal communication.
