Concord Hospital is seeking a Social Worker I to provide assessments and referrals for patients with complex care needs. Under supervision, you will help families adjust to illness and preserve patient self-determination in decision making.

The role requires 3–5 years of clinical social work experience, preferably in acute care, and a Bachelor’s degree in Social Work or related field. Join a team dedicated to quality patient care and collaborative planning.

Qualificações

• 3 to 5 years of Clinical Social Work experience required.
• Acute Care Social Work experience preferred.
• Bachelor's degree in Social Work or related field required.

Responsabilidades

• Conducts a comprehensive assessment of patients/families' behavioral, psychological and socioeconomic needs.
• Identifies resources needed to adapt to illness throughout the continuum of care.
• Attends Care Planning or Discharge Planning rounds to facilitate communication and align with patient/family goals.
• Participates in advance care planning initiatives.
• Documents measurement and evaluation of social service consultation and outcomes as progress toward goals.
• Collaborates with team using leadership and communication skills.
