Job summary

Concord Hospital in New Hampshire is seeking a Social Worker I to assess complex care needs and coordinate referrals. You will support patients and families through illness adjustment and crisis intervention, with emphasis on preserving patient self-determination in decision making.

Under supervision, you will apply NASW ethics, document progress, and collaborate with Care Management and care teams to align goals with patient needs and preferences.

Qualifications

• 3 to 5 years of Clinical Social Work experience required.
• Acute Care Social Work experience preferred.
• Must have either bachelor's degree in social work or related field.

Responsibilities

• Conducts a comprehensive assessment of the patients/families' behavioral, psychological and socioeconomic needs.
• Identifies resources needed to adapt to illness throughout the continuum of care.
• Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance of the patient and families goals.
• Participates in advance care planning initiatives.
• Documents measurement and evaluation of social service consultation and satisfaction outcomes as patient progresses towards desired goals.
• Reviews decision making with Lead/SW II when appropriate; collaborates with team using leadership and communication skills.
