Responsibilities

• Conducts psychosocial assessments and develops care plans.
• Provides emotional support, counseling, and crisis intervention.
• Facilitates referrals to community resources and coordinates post-discharge services.
• Ensures seamless transitions and discharge planning with the healthcare team.
• Maintains EPIC documentation of assessments, plans, interventions and outcomes.
