Job summary

Bronx-Lebanon Hospital Center is seeking a Social Worker to provide psychosocial assessment, discharge planning and crisis intervention for inpatients and ambulatory care patients. The role requires MSW and New York State licensure.

You will collaborate with an interdisciplinary team, manage a patient caseload, document thoroughly, and participate in department and community activities to support patients and families.

Qualifications

• Master's in Social Work (Required).
• New York State Licensed Social Worker (Legally Required).
• Computer literacy (Required).

Responsibilities

• Establishes and maintains positive relationships with patients, visitors and employees.
• Carry an active caseload of patients/families per department policies.
• Provide bio-psychosocial/cultural assessment and treatment; crisis intervention as indicated.
• Collaborate with the inter-disciplinary team in patient care.
• Provide reports, records documentation and statistics as required.
• May serve as liaison within the department, hospital and to community agencies.
• May delegate in-service training and represent social work in meetings and conferences.
• Perform other activities to assure services to patients and the functioning of the Social Work Program.
