Job summary

Brown University Health is seeking a Social Worker/LSW to evaluate, diagnose and treat psychosocial problems, assisting patients and families in overcoming barriers arising from illness to restore optimal functioning.

The incumbent will participate in the co-management model with unit leadership to support patient flow, provide education, and collaborate with multidisciplinary teams while upholding NASW ethics and Brown's values of compassion, accountability, respect and excellence.

Qualifications

• Master's Degree in Social Work from an accredited graduate school.
• Current license from State of RI, Board of Social Work Licensure.
• Demonstrates eligibility for LICSW status (two years or 2000 hours of post-master supervision) or LICSW.

Responsibilities

• Completes bio-psychosocial evaluations to identify barriers to treatment and discharge.
• Provides consultation regarding patient background and social work needs to the care team.
• Develops treatment plans and documents activity in medical records.
• Participates in team conferences to coordinate care and community reintegration.
• Provides education to patients and families about medical and social work recommendations.
• Supervises field placements for BSW/MSW candidates (LICSW or 5+ years experience).
